Dreamweavers Supported Living
Dreamweavers, Inc. is a non-profit in Madison, WI founded in 1993. As a supported living provider we offer direct assistance to individuals with varying disabilities living in their own homes throughout our community.
Our main roles are supplying overall support coordination/facilitation and direct staff to help each person carry out all aspects of independent living as needed: personal care, medication administration, monitoring health and accessing quality healthcare, home upkeep, meal prep, financial management, errands, socialization, mentorship, emotional guidance, fostering relationships, activity engagement inside and outside home, as well as specific skill building as desired.
Our progressive and diverse organization values individualized approaches to daily support, as we work in tandem with the person and wider team towards goals promoting self-determination. Our agency strives to provide the person centered support essential for each individual to experience a safe and happy life which contributes to strengthening our entire community.
